Tons of pulls after washing 3 times

Pretty disappointed. Got matching ones for my daughters in their room. But, after using the sheet, washing maybe 3 times, it has a ton of runs in the fabric/ pulls. One is even starting to have a small hole. I will also say that it's pretty loose on the crib mattress. My oldest I 2 so I feel fine if she uses it, but I would be a little nervous for little ones.
